Description (Service) The Ontario Photo Card is a government-issued identification card available to residents of Ontario, 16 years of age and over who do not have a driver's licence  
 
Photo Card has security features similar to a driver's licence, to make it easier for people who do not have a driver's licence to use as a valid proof of identification, open a bank account, and perform any other activities that require official ID

Application In person at applicable ServiceOntario Counters * see website for locations currently offering this service * bring original identity documents to prove your legal name, date of birth and signature and marriage certificate if applying under married name 
 
Card will be mailed in 4 to 6 weeks. 
 
If individual has no identity documents they can obtain a Declaration from a Guarantor form
