| Hours | Mon-Thu 9 am-5 pm, Fri 9 am-4:30 pm plus one evening a week for appointments if needed. |
| Areas Served | North Perth ; Perth County ; Perth East ; Perth South ; St Marys ; Stratford ; West Perth |
| Eligibility | Youth at high school age and adults 18 and over with Developmental Delay and/or caregivers |
| Application | Self-referral and agency referrals accepted - call office and ask for intake. |
